This concerns first name, middle name, last name, date of birth, gender, address, telephone number, e-mail address, health insurance details, citizen service number (BSN), health data, other personal data that you actively provide, for example in correspondence or by telephone.

We process your personal data only for the following purposes:
We are required by the Medical Treatment Agreement Act (WGBO), the Health Insurance Act (Zvw) and the Act on the Use of Citizen Service Numbers in Health Care (Wbsn-z) to establish your identity, keep a medical record and register health insurance information. To do this, we need, among other things, your name, date of birth, BSN, proof of identity and insurance information. If you do not provide these details, we will unfortunately not be able to treat you, because we would be acting in violation of applicable legislation.

Personal data is not kept longer than necessary. We keep medical records for 20 years in accordance with government guidelines. We keep invoice data for at least 7 years in connection with tax obligations.

You can always unsubscribe via a link at the bottom of the newsletter. The data transfer to the US takes place on the basis of standard contractual clauses in accordance with the AVG.
